A good rehab program deserves a good landing. Our commitment: continuity, the gains you made as an inpatient protected by supports that start the day you walk out.
Discharge isn’t one-size-fits-all. We build the option that’s actually safe:
Call from the ward, the waiting room, or home, we take it from there.
The earlier we’re in, the smoother it goes. We’ll assess needs and liaise with the treating team same-week.
Supports, housing, equipment and therapy organised in parallel honest timelines shared with everyone.
Supports start on discharge day then we review in the first weeks and adjust as home life settles in.
Referring from Sydney Southwest Private? Send the referral, we’ll respond within one business day with what we can mobilise, and honest timeframes for the rest.
We accept complex, high intensity and psychosocial discharges, including participants without safe housing.
NDIS-registered across the support types discharges actually need, coordination, daily personal activities, high intensity, accommodation and therapeutic supports with one contact point for your whole referral.
You don’t have to organise a discharge alone. Call us, we’ll explain what can be arranged, work alongside the The Hills Private team, and keep you and your family informed at every step.
The best discharges are planned early. One call today means supports, housing and therapy ready when your team says you’re ready.

Sydney Southwest Private runs inpatient rehabilitation programs with a set end date which makes discharge planning easier to get right. We coordinate with your rehab team before program end, so community therapy, supports, equipment and (if needed) interim housing start the day the program finishes.
We start planning the day you call and can often mobilise care workers and coordination within days. Honest caveat: some elements, NDIA plan changes, equipment orders, housing have their own approval timeframes. We move everything we control fast and give you straight answers on the rest.
That’s one of the most common reasons discharges stall and something we specifically solve. Short Term or Medium Term Accommodation (STA/MTA) can bridge the gap while home is modified or a longer-term option like Supported Independent Living is arranged.
Yes. A hospital stay is often when disability support needs first become clear. We help you and your family understand NDIS access, gather the evidence the application needs, and work with the treating team, so the process starts before you’re home.
Yes, that’s the point of a planned discharge. Our psychology, OT, speech and behaviour support teams pick up where the inpatient program finished, and we coordinate with your rehab clinicians so nothing gets lost in the handover.
Your NDIS plan different pieces draw on different budgets: Core funding for daily care, Capacity Building for coordination and therapy, and dedicated STA/MTA funding for interim accommodation. We’ll map your plan and tell you exactly what’s covered before anything starts.
Yes, Liverpool, Casula, Moorebank, Prestons, Glenfield, Ingleburn, Campbelltown and the wider region. If you’re going home somewhere else entirely, ask between local teams and telehealth therapy we cover more ground than this list.
Ongoing supports and therapy are exactly what we set up and please know discharge support isn’t a crisis service. If you’re in crisis or at immediate risk, call 000, or Lifeline on 13 11 14 (24/7). For everything else, call us on 1300 615 193.
Understanding your first NDIS plan is easier when you know how each budget works and where different supports are funded. Three budgets, unfamiliar category names, different flexibility rules and real money that only helps if you know how to use it. Here’s the plain-English version. (Completely new to the scheme? Understanding the NDIS can feel like learning a new language, especially when you encounter terms like NDIS Improved Daily Living, CB Daily Activity supports, or NDIS capacity building. Many people hear “Improved Daily Living” and assume it simply means getting day-to-day support. In reality, its purpose is completely different. NDIS Improved Daily Living sits within Capacity Building […]
