Westmead is one of NSW’s largest hospitals, and its wards discharge everything from major trauma to long-stay complex admissions. We work with the treating team and ward social workers, map your NDIS funding, and organise supports, equipment and where needed interim housing so discharge isn’t delayed.

We start planning the day you call and can often mobilise care workers and coordination within days. Honest caveat: some elements NDIA plan changes, equipment orders, housing have their own approval timeframes. We move everything we control fast and give you straight answers on the rest.

That’s one of the most common reasons discharges stall and something we specifically solve. Short Term or Medium Term Accommodation (STA/MTA) can bridge the gap while home is modified or a longer-term option like Supported Independent Living is arranged.

Yes. A hospital stay is often when disability support needs first become clear. We help you and your family understand NDIS access, gather the evidence the application needs, and work with the treating team, so the process starts before you’re home.

Yes, we’re registered for high intensity daily personal activities (tube feeding, wound care, catheter management and more) and provide Specialist Support Coordination (Level 3) for high-risk transitions, including psychosocial discharges.

Your NDIS plan, different pieces draw on different budgets: Core funding for daily care, Capacity Building for coordination and therapy, and dedicated STA/MTA funding for interim accommodation. We’ll map your plan and tell you exactly what’s covered before anything starts.
